INFORMED CONSENT FOR IN-PERSON MEETINGS DURING THE COVID-19 PANDEMIC
This document contains important information about the 2021 Annual Planning Meeting of the Retirement Advisor Council being held in-person July 12-14 at Lincoln Financial Field in Philadelphia, PA, during the COVID-19 public health crisis.

Clicking “accept” at the end of this agreement, signifies that you agree to waive and release EACH Enterprise, Lincoln Financial Group, Lincoln Financial Field and their respective directors, officers, employees, attorneys, affiliates and subsidiaries, from any and all claims, offsets, defenses and counterclaim, such waiver and release being with full knowledge and understanding of the circumstances and effect thereof.
UPDATE AS OF JULY 6, 2021: The requirement to wear a mask covering your nose and mouth in all areas of the in-person event except when eating or drinking is waived for all attendees vaccinated against Coronavirus.
Decision to Meet In-Person
The meeting planning committee of the Retirement Advisor Council has agreed to hold the July 2021 Annual Planning Meeting as a hybrid, giving you the option to attend online or in-person. If you have concerns about meeting in-person, or about traveling to attend the meeting, you are encouraged to attend the meeting online. The decision to meet in person, and to travel to attend the meeting is yours, and yours only.
Jurisdiction
You understand that the meeting is subject to regulations and ordinances of the City of Philadelphia and the Commonwealth of Pennsylvania. Should the City of Philadelphia or Commonwealth prohibit indoor gatherings over a certain size, we reserve the right to cancel the in-person meeting without consent, notice, or indemnification. Sole Discretion
At its sole discretion, EACH Enterprise reserves the right to cancel any part of the in-person meeting without consent, notice, or indemnification if it believes meeting in-person presents an undue danger for attendees, staff, or others involved in the organization of the meeting.
Risks of Opting for In-Person Meeting
You understand that attending the in-person meeting and traveling by public transportation or ridesharing to attend the meeting, you may be exposed to coronavirus and other public health hazards.
Your Responsibility to Minimize Your Exposure
As a condition of in-person attendance, you agree to take the following precautions which will help keep everyone safer from exposure. If at any time, you do not adhere to these safeguards, you will be refused attendance.
- Get vaccinated for Coronavirus
- Refrain from attending if you display common symptoms of COVID-19, including
- Fever over 100.4 Fahrenheit
- Dry cough
- Tiredness
- Aches and pains
- Sore throat
- Diarrhea
- Conjunctivitis
- Headache
- Loss of taste or smell
- Skin rash or discoloration of fingers or toes
- Difficulty breathing or shortness of breath
- Chest pain or pressure
- Loss of speech of movement
- Refrain from attending if you have been exposed to others who are infected by COVID-19 in the 10 days before the event
- Submit to a temperature check by staff upon entering any event associated with the meeting
- Wash your hands or use alcohol-based hand sanitizer when entering the meeting space.
- Adhere to the safe distancing precautions set up in the event space.
- Wear a mask covering your nose and mouth in all areas of the in-person event except when eating or drinking (All staff will too).
- Keep a safe distance of 6 feet or more when unmasked
- Avoid shaking hands
- Follow COVID-19 guidelines from the Center for Disease Control (CDC) posted HERE
The above precautions may change to comply with local, state, or federal orders or if warranted by guidelines from health organizations. Changes will be communicated to attendees before and at the meeting.
If You Are Sick During the Meeting
EACH Enterprise is committed to keeping people safe from the spread of the COVID-19 virus. If at any time during the event, you display symptoms of coronavirus, you will be required to leave the meeting immediately, and encouraged to get tested for Coronavirus. Notifying us of test results will help us prevent further spread of the disease.
Contact Tracing
If we believe you may have been in contact with Coronavirus at the meeting, we will notify you via text message. We encourage you to get tested for Coronavirus. Notifying us of test results will help us prevent further spread of the disease.
Your Confidentiality in the Case of Infection
If you test positive for coronavirus, we may be required to notify local health authorities that you attended our in-person meeting. We will only provide the minimum information necessary to comply with regulations.
Precautions We Will Take To Keep You Safer at the In-person Meeting.
EACH Enterprise is taking the following precautions to protect Council Members, Speakers, Staff, Venue Staff, and others to help slow the spread of the coronavirus.
- Meeting seating arranged for appropriate physical distancing
- Access to bathrooms controlled to support safe distancing
- Hand sanitizer available in all venues
- Temperature checking at venue entrances
- Periodic wiping of surfaces during the meeting
- Meeting space cleaned at the end of each day
- Council and venue staff will wear masks at all times (except when eating and drinking)
- Council and venue staff maintain safe distancing
- Attendees and speakers will be encouraged to keep a safe distance and to always wear a mask (except when eating or drinking).
- Attendees and speakers to wash or sanitize hands at venue entrances
- Handshaking discouraged
- Staff encouraged to be vaccinated
- Access to bathrooms controlled to support safe distancing
- Tissues and trash bins easily accessible.
